Items Needed for Change of Address/ Proof of Residency

Due to the Federal Court Order mandate that we are presently under we are required to have proof of residency on file for every student.  Anytime a student moves/changes address we must have the following information presented by the parent or legal guardian of the student immediately for proof of residency.  Only the parent/legal guardian can make these changes.

Proof of residence must include ALL 3 of the following:

1) A copy of parent/legal guardian's most recent Alabama Power Company bill.  Sorry...we can not accept any other utility bill.

2) A copy of parent/legal guardian's most recent rent, lease or mortgage agreement (with address included).

3) You will also be required to complete an Affidavit of Residency.

Parent/legal guardian must have a picture ID, and custody information in writing if divorced or applicable, to make change

If you are not living within our school zone, as per the Federal Court Order, your child must be withdrawn to attend the school which corresponds to your residence.
