Withdrawal Form from the Previous School

(Must include Grade Placement, Grades, Attendance & Discipline)

Original Blue - Alabama Immunization Form

(Must be current with ALL shots up to date)

Copy of Certified Birth Certificate

Copy of Social Security Card

*Proof of Residency

(MUST include ALL of the following)

*1) Alabama Power Bill

(This is the ONLY utility bill that we can accept)

*2) Mortgage, Rental or Lease Agreement

(Must show guardians name and address)

*3) Notarized Affidavit of Residency

(This is provided by us and can be notarized here in our office)

*Guardian must have proper Picture ID

and Legal Documentation/Custody papers if divorced/applicable

(These legal documents MUST be issued through

Family Court with a Judges Signature and Case Number included.

We CAN NOT accept Power of Attorney!)
